Introduction
Hip arthroscopy for femoroacetabular impingement syndrome (FAIS) has long been viewed as a procedure yielding better outcomes in younger patients. However, groundbreaking new research suggests that age may not be as limiting as once thought. This study done by Dr. Okoroha’s colleagues explores how patients aged 40 and older fare after hip arthroscopy, comparing their outcomes to younger counterparts over a minimum 10-year follow-up period. The findings challenge assumptions and highlight the durability of this surgical approach across age groups.
Study Highlights
Patient Demographics: Researchers studied 53 patients aged 40 years and older (mean age 48.3 years) and matched them 1:1 by sex and BMI with 53 patients younger than 40 years (mean age 28.9 years). Both groups had comparable preoperative characteristics and radiographic findings.
Follow-Ups: Patients were assessed for at least 10 years postoperatively using validated patient-reported outcomes (PROs) and clinical benchmarks such as MCID and PASS. Rates of secondary surgery, including revisions and conversions to total hip arthroplasty (THA), were also evaluated.
Positive Outcomes: Both age groups experienced significant improvements across all PRO measures, with high rates of MCID and PASS achievement. There were no significant differences in postoperative improvement scores between cohorts.
Key Findings on Risks: Rates of complications, revisions, and THA conversions were low and comparable across age groups. The older cohort demonstrated similar overall survivorship to their younger counterparts, with no significant differences in long-term durability of surgical outcomes.
Conclusion
This study reinforces that age should not be a deterrent for patients considering hip arthroscopy for FAIS. Individuals aged 40 years and older can achieve outcomes on par with younger patients, enjoying significant pain relief and functional improvement over the long term. These findings advocate for expanding access to hip arthroscopy, empowering older patients with FAIS to seek effective treatment without the fear of age-related compromise.
